Thanks to 88hk's recent pictures, I think this house is Bracken Hill, 12 Barker Road. 

Based on its seemingly close proxmity to Hopewell Centre, I had speculated earlier that this was the former house at 30 Peak Road.

If the house was not actually demolished, I think it would still be considered the same property and the occupation permit date might not have been changed in the land registry. 

In recent pictures you can see from the roof where the extension to the front of the house was made.
