Anthony, the marker on the map Annelise pointed you to will get you to the right area, but from memory the plaque is a short distance north-east of the marker. There's a path that runs from Wong Nai Chung Road to the Jockey Club Headquarters and Sports Road. You'll see the plaque if you walk along that path. Note that it's on the ground, not on a building.
